Asynchronous Treatment Consent

Please read this consent carefully before using Halftime Health’s clinical evaluation services. By submitting your intake form or clicking “I consent,” you acknowledge that you have read, understood, and agree to the terms described below.
1. What Is Asynchronous Telehealth?
Asynchronous telehealth (also called “store-and-forward” care) is a model in which clinical evaluation occurs without a real-time appointment. You submit health information — including your medical history, current medications, symptoms, goals, and photographs or other data as requested — and a licensed clinician reviews that information and responds, typically within 24–48 hours.
This differs from synchronous telehealth (live video or phone visits), in which you and a clinician communicate in real time. Halftime Health’s standard initial evaluation pathway uses the asynchronous model.
2. How Your Evaluation Works
After you complete your digital intake form, a licensed clinician will:
- Review your self-reported health history, current medications, and stated goals.
- Apply clinical judgment to determine whether the requested protocol is medically appropriate for you.
- Make one of the following decisions: (a) issue a prescription if clinically appropriate, (b) request additional information or documentation, (c) recommend a synchronous (real-time) visit for further evaluation, or (d) decline to prescribe if the requested protocol is not clinically appropriate.
You will receive notification of the clinician’s decision by email and through your account dashboard.
3. Clinician Authority and Independence
All prescribing decisions are made solely by the licensed clinician assigned to your case. Halftime Health does not direct, override, or influence clinician prescribing decisions. Clinical decisions are based on your self-reported information and the clinician’s professional judgment; they do not constitute an ongoing treating relationship unless expressly established.
The clinician may prescribe a different protocol, dose, or formulation than the one you requested, or may decline to prescribe entirely, based on their independent clinical assessment.
4. Limitations of Asynchronous Care
You understand and acknowledge that asynchronous telehealth has limitations, including:
- The clinician cannot perform a physical examination, observe you directly, or order diagnostic tests as part of the initial asynchronous evaluation (unless lab results are submitted as part of your intake).
- Clinical decisions are based entirely on the accuracy and completeness of the information you provide.
- Response times may vary; same-day or urgent care is not guaranteed through this pathway.
- Certain conditions or requested protocols may require a synchronous visit or referral to an in-person provider.
5. This Is Not Emergency Care
Halftime Health’s asynchronous care platform is not appropriate for emergencies, urgent medical conditions, or situations requiring immediate diagnosis or treatment. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room. If you are having thoughts of self-harm or suicide, contact the 988 Suicide and Crisis Lifeline by calling or texting 988.
6. Your Responsibilities
By using this service, you agree to:
- Provide complete and accurate information in your intake form, including all current medications, supplements, allergies, and relevant health conditions.
- Notify us immediately of any changes to your health status or medications after submitting your intake.
- Follow any instructions or recommendations provided by the clinician, and seek in-person care if directed to do so.
- Not use this platform as a substitute for emergency care or for conditions requiring immediate attention.
7. Accuracy of Information
You represent and warrant that all information you provide is true, accurate, and complete to the best of your knowledge. Providing false or misleading health information may result in clinically inappropriate prescribing and poses serious risks to your health. Halftime Health and its affiliated clinicians are not liable for adverse outcomes resulting from inaccurate or incomplete information you provide.
8. Voluntary Consent
Your participation in asynchronous care is voluntary. You may request a synchronous visit or seek in-person care at any time. You may also withdraw from the platform at any time, subject to the terms of your subscription agreement.
