Request for Release of Laboratory Results
Request and Authorization
I hereby request that Labcorp (and any other laboratory service provider used in connection with my Halftime Health membership, including Walgreens as a collection site partner) release to me — via Halftime Health, LLC, as my authorized agent — all completed test results arising out of my receipt of laboratory services from Labcorp (or such other laboratory), as accessed via Halftime Health, LLC (the "Results").
What This Authorization Covers
This request and authorization applies to:
- All completed blood panel results from my Halftime Baseline lab draw (Panel TRT001 for male members, Panel HRT002 for female members), and
- Any subsequent lab panels ordered in connection with my Halftime Health membership, including quarterly re-test panels.
My Results will include biomarker values for the approximately 40+ biomarkers measured as part of the Halftime Health blood panel, which may include: complete blood count (CBC), comprehensive metabolic panel (CMP), lipid panel, hormone markers, thyroid markers, inflammation markers, metabolic markers, growth and longevity markers, and nutritional markers.
Acknowledgments
I hereby acknowledge and accept the following:
(a) Physician ordering and receipt. Either prior to or simultaneously with the communication of the Results to me, the Results will be communicated to the licensed physician or physician group who ordered the laboratory test(s) (the "Ordering Physician"). In connection with Halftime Health's services, the Ordering Physician is a licensed medical professional within OLA Digital Health's contracted physician network (or a successor contracted physician network provider). The Ordering Physician reviews my Results as part of the physician-supervised protocol review process.
(b) Not a substitute for medical care. The communication of the Results to me should not be viewed as medical advice and is not meant to replace direct communication with the Ordering Physician or my primary care provider. While Halftime Health facilitates access to my Results through its member platform and provides a biomarker analysis as a clinical decision support tool for the Ordering Physician's review, this analysis is not a diagnosis and does not constitute medical advice.
(c) Interpretation of Results. All clinical inquiries regarding the meaning, interpretation, or medical significance of my Results should be directed to the Ordering Physician, my primary care provider, or another qualified healthcare professional. Halftime Health's biomarker analysis and personalized protocol plan are generated as a physician review tool — the Ordering Physician makes all final clinical determinations, including whether to approve or modify any proposed protocol.
(d) No referral fee or consideration. The communication of the Results to me does not include any information that would constitute consideration given for referrals, including medical advice specifically directed at me concerning my condition.
(e) Compounding pharmacy context. I understand that, following physician review and approval of my personalized protocol plan, the Ordering Physician may issue prescriptions for compounded peptide preparations. My Results may be used by the Ordering Physician in connection with writing such prescriptions. Compounded peptide medications are patient-specific preparations made by licensed 503A compounding pharmacies (Wellsync/BoomRx and/or MediVera Lifefile); they are not FDA-approved drug products, and individual results may vary. I further understand that the regulatory status of certain compounded peptides — including BPC-157, TB-500, CJC-1295, KPV, and MOTS-C — is subject to active FDA review: these substances were removed from FDA's Category 2 restricted list in or around April 2026 but have not been added to the Category 1 authorized list for 503A compounding. The FDA Pharmacy Compounding Advisory Committee is scheduled to consider their status in July 2026. The availability of specific peptide protocols may therefore change based on these regulatory developments, and my Ordering Physician and Halftime Health will notify me of any material changes affecting my protocol.
(f) Direct lab access. I understand that, in addition to accessing my Results through the Halftime Health member platform, I may have the right to access my Results directly from the laboratory under applicable law (including CLIA and any applicable state law). I may contact Labcorp directly for information about direct access to my records.
(g) Data handling. My Results are PHI (protected health information) and will be handled by Halftime Health in accordance with HIPAA, Halftime Health's Privacy Policy, and the applicable Business Associate Agreement between Halftime Health and its physician network, laboratory partners, and pharmacy partners.
Revocation
I may revoke this request and authorization at any time by contacting Halftime Health in writing at legal@halftime.health. Revocation will not affect prior releases already made in reliance on this authorization.
